Advances in genetic testing have enabled the diagnosis of IBD caused by gene mutations, also known as monogenic or Mendelian disorder-associated IBD (MD-IBD), with approximately 60 causative genes reported to date. The diagnosis of VEO-IBD requires endoscopic and histological evaluations. However, satisfactory small bowel imaging studies may not be feasible in this small population. Both genetic and immunological approaches are necessary for the diagnosis of MD-IBD, which can differ among countries according to the available resources. As a result of the use of targeted gene panels covered by the national health insurance and the nationwide research project investigating inborn errors of immunity, an efficient approach for the diagnosis of MD-IBD has been developed in Japan. Proper management of VEO-IBD by pediatric gastroenterologists constitutes a challenge. Some MD-IBDs can be curable by allogenic hematopoietic stem cell transplantation. With an understanding of the affected gene functions, targeted therapies are being developed. Social and psychological support systems for both children and their families should also be provided to improve their quality of life. Multidisciplinary team care would contribute to early diagnosis, proper therapeutic interventions, and improved quality of life in patients and their families.This case series of 5 patients of rigid bronchoscopy done for tracheobronchial foreign bodies is presented to readers to share my experience of doing rigid bronchoscopy during Covid Times from March to July 2020 specifically sharing experience on personal protection with local innovations during the procedure. It’s still an ongoing pandemic with greater morbidity and mortality. In our study, we describe clinical features, and demographics of patients presenting to our tertiary care center with Covid-19 infection. Global burden was around 14 million Covid-19 positive cases with .5 million deaths and rising as per WHO update. A descriptive analytical cross-sectional study was carried out from June 1st to 30th, 2020 in a tertiary care center. Patients who tested positive by Reverse Transcriptase-Polymerase chain reaction were included in the study and clinical characteristics and outcome of the patients were analyzed along with impact on quality-of-life. Sample size was 250. Mean age was around 41 years, with age range from 15 to 75 years. Gender ratio was 12.4, with male preponderance. Most common presenting symptom was throat pain (60%). Rhinorrhea and sneezing complaints were not recorded. Clinical categorization was done and scored accordingly. Clinical outcomes of above patients showed complication rate 5.2%, mortality rate .8%, with discharge and recovery rate being rate 94.8% and 96% respectively. Fear of COVID-19 scale was used to assess quality-of-life impact. We found throat pain was the most common symptom in our study. No cases reported with rhinorrhea/sneezing. Good recovery rate noted. Therefore contingency plans are required for dealing with patients in outpatient clinics, and while performing diagnostic endoscopies, minor procedures in OPD and surgeries in operating rooms. Infected patients may shed severe acute respiratory syndrome coronavirus-2 particles into their environment via body secretions. Therefore, Otolaryngologists should be vigilant. In this submission, we share our experience of an innovative practice plan in redesigning the ENT OPD setup, endoscopy set up and OT so as to reduce the risk of transmission of virus not only to doctors but other healthcare workers.
